Shipping instructions

I need to add a 'special shipping instructions' box to the delivery address form, where customers can write for example 'if out, leave parcel with my neighbour'..

Can anyone tell me if this is possible, and if so, how do I do it?!

i'm working on multiple added field and an extra step for checkout already working at this place

(extra date / time / comment & text field working)

for instance field can only be edited in extra step and reported in summary
hope i'll have later any field selected or edited in any part of the checkout

Thanks for your response - I dont quite understand though....
I just essentially want to add a box below the telephone number field on the address form - but can't work out which file to edit in order to do this...
Any ideas anyone??
