menu and template choice for new page

It must be possible to add these fields to the add new page block so that you can select in whicht menu the new page has to be...

I have been fooling around with the files in de admin/pages folder and I managed to display the extra fields but the foldoutmenu's in which the menu options and the template options have to be displayed I get nothing...

anyone with a solution?
