problem: form modul v3.2.14 does'nt show the titles

I have installed WB 2.13.2 r133 for a website.
When I create a form, the titles are not shown, only the fields.
The heading is shown.

What is the problem here an how I can fix that?

Set an output template in the Form module under Option.
None will have been assigned yet.

But why isn't this preset, or WB shows an error?

Thanks for your help hgs, but maybe I'm to stupid to find the right place to set an output template.
Where can I find and change this setting?
Please, can someone stick my nose to it.  :-o
Today I upgraded to v2.13.3 r166 and experienced the same problem like gba.
I searched the backend up and down and couldn't find an option button neither in addons or options nor in form section.
I'm deeply sorry for my inconvenience.

Did you load and view the attached PDF in hgs' post?

Use the SETTINGS-Button in the form module. Scroll down to the block "Layout Settings"

in the top of this block, you've a select-field, called "Layout file name". Choose here one of them from the list. Now click on the Button "IMPORT".
You see then the code of this output-template in the follow textfields from the next block, called "Layout Output"

Maybe its good, if you try at first the Template "DefaultTableLayout". these are the default values from older WB-Versions
