How can I add an additional field to the Submit Order

I found what I think is the form for the Bakery Submit Address form under : templates/form/form.htm  -  I attached a screen shot of the page I am talking about.

What I need to do though is to add another single line field at the top of the form for STUDENT NAME.  I can't seem to figure this out though.  Can someone please tell me how to do this.  I just need this in the form since the person paying for the items will not always be the student and we need to know who the purchase is going to be for during the event.

Changing things like this is almost impossible in Bakery.
Adding fields in the form is one, but the code will need to read/save/use the data too.

I would suggest to try and rename the text for "First name" and "Last name" into "Student name" and "Full name" (or something similar)

Darn... The First and Last name is for the person buying the product, which in this case is most likely a parent, so I need to figure out a way to have the student mentioned in the purchase.
